Background of the Study
Advanced care planning (ACP) involves discussions between healthcare providers and patients about future care preferences, particularly when patients are facing life-limiting illnesses. This process is critical for ensuring that patients' wishes are respected and that they receive appropriate care during the final stages of life (National Hospice and Palliative Care Organization, 2024). Nurses, who are central to patient care, are often the first point of contact in initiating ACP conversations, offering guidance, and ensuring that patients' preferences are documented and followed.
At General Hospital, Minna, there is an increasing need for structured ACP processes, especially as the population ages and more individuals are diagnosed with chronic, terminal illnesses. However, despite its importance, there is limited understanding of how nurses engage in ACP and the barriers that hinder their involvement. This study seeks to evaluate the role of nurses in facilitating advanced care planning for terminally ill patients at General Hospital, Minna, and to identify strategies to enhance their involvement.
Statement of the Problem
Advanced care planning is a crucial aspect of end-of-life care, but the lack of structured processes and training in ACP may limit nurses' ability to engage patients in these important conversations at General Hospital, Minna. This study will investigate the extent to which nurses are involved in ACP and explore how their participation can be enhanced to improve care outcomes for terminally ill patients.
Objectives of the Study
1. To evaluate the role of nurses in facilitating advanced care planning for terminally ill patients.
2. To identify barriers to effective nurse involvement in advanced care planning.
3. To recommend strategies for improving nurse participation in advanced care planning for terminally ill patients.
Research Questions

Research Hypotheses

3. Structured training and support will increase nurses' involvement in advanced care planning, improving patient care outcomes.
Scope and Limitations of the Study
This study will focus on the role of nurses in advanced care planning for terminally ill patients at General Hospital, Minna. Data will be collected through surveys and interviews with nurses, patients, and their families. Limitations may include reluctance from patients to engage in ACP and the variability in nurses' experience and training.
